We are seeking experienced care coordinators to provide culturally appropriate home care to older individuals or those with disabilities and their carers. You will assess unique needs, develop personalized care plans, and collaborate with clients, healthcare professionals, and stakeholders to enhance their quality of life and maintain independence. Prior experience in care coordination is preferred, but candidates with a passion for caregiving and a willingness to learn are also welcome.
