We are seeking a passionate and skilled Community Nurse to provide high-quality care to individuals and families in our community. This role involves delivering clinical services, health education, and support to clients in their homes or within the community, promoting health and independence.

Key Responsibilities:

  • Provide comprehensive nursing care, including assessments, treatments, and medication management.
  • Support patients with chronic conditions, post-operative care, and palliative needs.
  • Collaborate with multidisciplinary teams to develop and implement individualised care plans.
  • Promote health and well-being through education and preventative care initiatives.
  • Maintain accurate documentation and adhere to compliance and professional standards.
  • Build strong relationships with clients, families, and community organisations.

Requirements:

  • Current registration with [Insert relevant nursing body, e.g., AHPRA.
  • Bachelor’s degree in Nursing or equivalent qualification.
  • Previous experience in community or primary healthcare (preferred).
  • Excellent communication, organisational, and interpersonal skills.
  • Valid driver’s license and access to a reliable vehicle.
  • A genuine passion for providing holistic care in a community setting
